Dear participant,

You are being invited to participate in the “Development, validation and application of a game for teaching the physiology of the hypothalamic-pituitary axis for undergraduates of the courses in the areas of health.”, developed by Letícia Corrêa Vaz, Master’s student in Physiological Sciences at the Federal University of Pampa - UNIPAMPA, under the guidance of the Professor Dr. Mauren Assis de Souza, which aims to develop, validate and apply a educational game for teaching the physiology of the hypothalamic-pituitary axis.

Through this document and at any time, you may request clarifications additional information about the study in any aspect you wish. You can also withdraw your consent or discontinue participation at any time without suffering any kind of penalty or damage.

After being clarified about the following information, in case of accepting to be part of the study, sign at the end of this document, which is in two copies. One is yours and the other will be filed by the responsible researcher.

Your participation will consist of answering questions from a questionnaire to the project researchers. At the end of the survey, the questionnaires will be kept on file, for at least 5 years, according to CNS Resolutions 466/12 and 510/16 and guidelines from the CEP/Unipampa and at the end of this period, they will be discarded.

The risks for participating in the research will be minimal, as it is only a questionnaire in order to validate the game. However, you may feel uncomfortable responding to it. In this case, you can request to abandon the survey or stop filling out the form. questionnaire at any time. As it is a data collection research, without interventions, the participants will not receive follow-up after the execution of the project, however, they will be able to contact the researchers at any time by made available.

As a benefit for participating in the study, you will be able to know what is being newly produced in the area, you will be able to review specific concepts in your area and will have access to a report on the applicability of a game used for teaching. You can also, from of this, initiate a reflection on the subject. There will also be feedback to the scientific literature about the topic.

You will not incur any cost to participate in this study, nor will you receive any financial advantage. The expenses necessary for your participation in the research will be assumed by the researchers.

Your name and identity will be kept confidential, and survey data will be stored by the responsible researcher. The results will be published in scientific publications or presentations at events.

If there is any damage resulting from this research, you will be entitled to compensation, through judicial channels, as provided for in the Civil Code, the Civil Procedure Code, in the Resolution No. 466/2012 and Resolution No. 510/2016 of the National Health Council (CNS).

In case of doubt regarding the ethical conduct of the study, contact the Research Committee. Research Ethics at Unipampa. The Committee is formed by a group of people whose purpose is to objective to defend the interests of research participants in their integrity and dignity and thus, contributing to ethical standards in conducting research.
